The Most Measured Man Alive Still Missed This

Bryan Johnson tracks over 70 biomarkers. He has 30 doctors monitoring his biology around the clock. He spends millions annually optimising every measurable system in his body: cardiovascular age, inflammation markers, telomere length, all trending younger.
Last week, he announced he has autoimmune gastritis, a condition in which his immune system is destroying the acid-producing lining of his own stomach. It had been developing silently for over a decade. For 11 years, his ferritin (stored iron) was chronically low. His team tried food, tried supplementation, and nothing moved it. His own words: Low iron stores get normalized and rarely investigated at all when anemia hasn't shown up yet. That blind spot is what hid mine for a decade.
By the time an endoscopy confirmed early-stage autoimmune gastritis, the damage was already there.
That gap is worth looking at. Not to criticise Johnson, but because it illustrates something specific: biomarkers measure what the body is currently producing. They don't capture the trajectory of what's being depleted underneath. That's where a different framework is useful.
What is Autoimmune Gastritis
In autoimmune gastritis (AIG), the immune system attacks the stomach's parietal cells, the cells responsible for producing stomach acid. As those cells are progressively destroyed, acid production drops, and the stomach lining gradually thins and atrophies. The downstream consequences are significant: without stomach acid, the body can't absorb iron or vitamin B12 properly. This is why ferritin drops, and why supplementation doesn't fix it. The absorption mechanism itself is gone.
The condition is notoriously silent. It frequently develops over decades before producing symptoms obvious enough to investigate.

What TCM Sees: Liver-Stomach Heat Driving Stomach Yin Deficiency
In TCM, the stomach lining is the direct physical expression of Stomach Yin, its material substance. When it thins and atrophies, that is Stomach Yin Deficiency made structural. But Stomach Yin Deficiency isn't where the process starts. It's where it ends up after sustained heat has burned through the Stomach's fluid reserves over years.
The primary pattern here is Liver-Stomach Heat. A 2025 cross-sectional study of 122 AIG patients, classifying cases by TCM syndrome type, found that chronic and active inflammation were most severe in the liver-stomach stagnation-heat group, directly linking this pattern to the most aggressive tissue-level damage in autoimmune gastritis. Modern research on chronic atrophic gastritis of Stomach Yin Deficiency syndrome (YDCAG) further characterises the endpoint: inflammation of the gastric mucosa and atrophy of gastric glands, as the sustained heat progressively depletes the lining's material substance.
Johnson's ferritin wasn't a separate problem. It was the downstream readout of parietal cells depleted over years, destroyed by sustained Liver-Stomach Heat, leaving a stomach lining too thin and too depleted to produce acid or absorb iron. The low ferritin was the last signal, not the first.
